Attached is the order form.

ALL ORDERS MUST BE SUBMITTED TO PAULA CIMO IN PERSONNEL BY October 2, 2008, with a check made payable to Martin's Uniforms.

The Holiday and Super Bowl Deadlines are the same (Oct 2nd) because the company closes for an extended period around the holidays.

Keep in mind there is only one example of the badge on the order form - each badge will look like the corresponding rank ordered.  The officer's badge will look similar to the corporal badge with the Super Bowl seal versus the badge number/city seal.

Please e-mail Paula Cimo with any questions - and keep in mind this is a TPD only process in conjunction with the NFL for all sworn and non-sworn accordingly.
